Not having the right driver.
Plug the driver in and open “About this Mac”, “System Report”, “USB HI-Spped Bus”, “Hub”, & “USB-Serial Controller”. This will give the the information you need to search for the right driver. After downloading it, install.
Not having the cable pushed in far enough.
Sometimes this is hard. Others have cut the cable housing down, but I have never had to do this. I just work it way into the full “In” position.
Not letting the computer have enough time between steps.
This is the hardest step for me. After giving either the “Download from Radio” or the “Upload to Radio” command, I give a long 5 count between each further step. Sometimes it even me 10 seconds per step!
Using the above methods, I have gotten almost all of them programed in just a few minutes.
Good luck,

Cheap cables with bad prolific chips rarely work.
