For some reason your excel file (saved as a csv file) is omitting the last five columns after channel 16 (line 17) down.

One thing to remember is that sometimes, but not all, Chirp errors are actually more notifications and if you proceed anyhow Chirp will fix the entry with default values.

Also starting after 135 there are blank entrys. Looks like line # 136 to 137 are missing, also 139 and 140. I don't know if this your problem or not.
