Well if Miklor couldn't help you, there is a cable loopback tester. http://www.kc8unj.com/soft.html
It will scan your computer for COM ports and test the cable if it is a kind that sends TX and RX serial data over the same line. If not then you just need to jumper the TX and RX together. So as Jim says if the cable won't pass this test it won't work.

I've sent many there and they usually got things sorted out. The Icom 91/92AD is listed in the daily build as supported so you may have a cable or driver issue.
